Enhanced Recovery After Surgery (ERAS) is a modern, evidence-based approach designed to reduce surgical stress and accelerate recovery after major surgery. At South Asian Liver Institute, we are among the first centres to introduce and actively implement ERAS protocols in liver transplantation — redefining how transplant recovery is approached in our region.

Our ERAS pathway combines modern surgical science, advanced anaesthesia, critical care, nutrition, physiotherapy, and personalised rehabilitation into one seamless recovery programme.

Before, During & After Surgery

Before Surgery — Preparing the Body
Recovery starts before entering the operating room. Our pre-transplant optimisation programme focuses on nutritional support, protein supplementation, respiratory exercises, physiotherapy and muscle strengthening, frailty assessment, diabetes and metabolic optimisation, and patient and family education. This improves physical reserve and helps patients tolerate major surgery better.
During Surgery — Reducing Surgical Stress
Our intraoperative ERAS strategies minimise physiologic stress while maintaining safety and graft function. These include goal-directed fluid therapy, advanced haemodynamic monitoring, blood conservation strategies, temperature maintenance, multimodal pain management, and refined surgical techniques. Every step is carefully coordinated to support faster postoperative recovery.
After Surgery — Early Recovery Begins Immediately
The ICU is no longer a place of prolonged immobilisation. Our ERAS recovery pathway emphasises early extubation, early mobilisation, respiratory physiotherapy, early enteral nutrition, optimised pain control, reduced sedation burden, and structured rehabilitation. The first walk after transplant begins as early and as safely as possible.

Prehabilitation
Structured physical conditioning, nutritional optimisation, and respiratory training before surgery to improve the body's capacity to recover.
Advanced Anaesthesia
Goal-directed fluid therapy, haemodynamic monitoring, and multimodal pain management that minimises opioid use and supports early recovery.
Early Mobilisation
Physiotherapy begins in the ICU — the first walk after transplant is not delayed for days. Early movement reduces complications and speeds recovery.
Specialist Nutrition
Early enteral nutrition, BCAA supplementation, and liver-specific dietary management to restore muscle mass, energy, and immune function.
Donor Safety
ERAS principles are equally important for living donors. Better pain management, earlier mobilisation, and faster recovery ensure donors return to normal life sooner.
Long-Term Wellness
